Job Description

Cradlefin Consultants are recruiting for practitioners who want to work and learn as a team, who are warm and engaging with a sound understanding of the Healthy Child Programme and Safeguarding policy and practice. Our client have an exciting development within our service and have implemented The Maternal Early Childhood Sustained Home-visiting (MECSH) programme this year to the families in Haringey. The successful Health Visitor will be the named health professional, with responsibility for the planning and delivery of the HCP To report to and be line managed by a Team Leader/CPT

Main duties:

  • Responsible for identifying need within a family. To use child and family centred public health approaches to address health inequalities. This includes the promotion of good health and the prevention of ill health in the community and the implementation and evaluation of individualised programmes of care
  • Be prepared to work within the Health Visiting team and with other agencies in an appropriate and open manner
  • Manage junior members of staff, ensuring the completion of annual appraisals, 1-1s, and personal development of self and junior staff
  • Undertake evidence based child and family health assessments, and develop child/family
    health plans in partnership with children, families and carers. To refer and treat and review as
    appropriately
  • Identify, refer and support children with special educational needs or disabilities. To liaise with other agencies in health and social care to facilitate programmes of care
  • Support and contribute to the implementation of a single point of contact, service developments, standard setting, clinical audit and benchmarking, to deliver needs-led evidence based care that supports the local and national priorities and policies
  • Be the principal point of contact for allocated GP Practices in an area. To develop and deliver cooperative working arrangements with GP services that support healthy outcomes for children and their families, and facilitate partnership working
  • Be the principal point of contact for allocated Children’s Centres in an area. To develop and deliver co-operative working arrangements with Children’s Centres which supports healthy outcomes for children and their families, and facilitate partnership working across Children’s Centres and other partner organisations
  • Work with local statutory, private and voluntary providers, and users to develop and promote practice that is accessible and responsive to all section of the local community, ensuring that services are targeted to the most vulnerable groups and individuals to support social inclusion
  • Ensure that junior staff and self-keep up to date with mandatory training and appraisal
  • Deliver evidence based care, and to keep updated on health visiting practice
  • Support the retention and recruitment of staff
  • Work in keeping with WH ICARE values
  • Manage a caseload of families

Requirements:

  • Educated to degree level or equivalent
  • Health Visiting professional qualification, with entry onto the 3rd part of the NMC Register
  • Evidence of Continuing Professional Development
  • Willingness to develop and support management of change
  • Excellent communication skills: IT, written and oral
  • Ability to delegate work appropriately and safely
  • Ability to set personal objectives, and manage time, priorities and stressful situations
  • Management experience in a health service setting
  • Knowledge of the Healthy Child Programme
  • The ability to identify, respond to and evaluate family health needs, including the delivery of public health programmes
  • Understanding of the safeguarding process and child protection procedures, clinical governance arrangements and the ability to make decisions in these areas
  • Knowledge of infection control standards
  • Commitment to the development of excellent services, which focus on improved outcomes for children

Shifts will not normally operate beyond 9pm in the evenings and appropriate pay enhancements will apply. Staff will be consulted about the introduction of changes to shift patterns.
